Polices To Our Patients 


Thank you for being our patient and trusting us with your health.
Our practice is committed to try to provide you with the highest quality care, service, and access. In order help accomplish these goals; we have established the following practice policies. Please be
aware of these policies and know that they have been created with your best interest in mind.


Appointments
Appointments can be booked in person or over the phone between our normal clinic hours. We can see only one patient per appointment: please make separate appointments for each member of your family who needs care. Please arrive 10 minutes before your appointment time to register. For your benefit and the benefit of all our patients, we try to stay on schedule (though emergencies do sometimes interfere). We will keep you informed of all delays. We are working to further streamline the registration process.

Late Arrivals
If you are more than 15 minutes late for a scheduled appointment, it may be necessary to
reschedule your exam so as not to inconvenience other patients. Upon check-in, one of our medical assistants will inform you of the anticipated wait time. At this time, you may choose to wait while we work you back into our schedule or reschedule your exam.


Cancellations
Cancellations are required no later than 24 hours before your scheduled visit.


No Show Policy
Due to an unusually large number of patients who do not show for their appointments and who do
not call in advance to cancel those appointments, we have been forced to institute a “NO SHOW” policy. See or call our receptionist for a detailed “NO SHOW” policy.


Walk-Ins
A “walk-in” visit means that our schedule is full. The time allotted to you is taken from another
patient’s appointed time. A “walk-in” visit needs to be the point. Walk-in visits must require urgent
care, but are not life-threatening. An appointment is required for non-urgent visits (routine physicals, follow-ups, medication refills, and weight management).

 

Patient Forms​

Clifton Care Inc. has made it easier for first-time patients by offering health-related forms online. Please select the appropriate form(s), print them out and bring them to the office already completed. By offering you this option to print the forms at home in advance of your appointment, you will be able to complete them at your own pace. Please print the forms on white paper only.
